Why it matters
White matter is the brain's wiring: the tracts of nerve fibres that carry signals between regions, and tissue that changes with age. A large body of MRI research has already linked physical activity, cardiorespiratory fitness and exercise to grey matter, the tissue where processing happens, but far less attention has gone to whether the same relationship holds for the connections between regions. If fitness relates to healthier white matter, that would matter for how the brain communicates across a lifetime, so the authors set out to synthesise everything published on the question in healthy adults.
What they did
The researchers conducted a systematic review following PRISMA guidelines, searching PubMed and Scopus for peer-reviewed studies published before 5 July 2021 that examined physical activity, cardiorespiratory fitness or exercise, which they group under the label PACE, against white matter measures in healthy adults. Sixty studies met the inclusion criteria. They pooled results in meta-analyses, tested how much the studies disagreed with each other using Cochran's q, checked for publication bias with the Begg and Mazumdar rank correlation test, and ran a meta-regression to explore what drove any disagreement between studies.
What they found
Higher PACE levels were positively associated with global white matter volume, with a small effect (Hedges's g of 0.137), and with global white matter anomalies (0.182). The associations were stronger for local microstructure in specific tracts: the corpus callosum, the large bundle connecting the two hemispheres, showed an effect of 0.345, and the anterior limb of the internal capsule showed 0.198. All of these pooled associations were statistically robust. The overall picture is consistent: fitter, more active adults tend to show more white matter volume and better local tract integrity, with effects ranging from small globally to moderate in specific regions.
Where it fits
This review extends the exercise-and-brain literature beyond grey matter, where positive associations were already well documented, into the less-studied territory of white matter. It supports the idea that fitness relates to the preservation of brain structure across the lifespan rather than to one tissue type alone. It also leaves the important questions open: most of the evidence is correlational, so it cannot establish that exercise causes better white matter, and the authors explicitly call for research into which specific training parameters and which neurobiological mechanisms are responsible.
What it means for you
For a reader, this is a reason to think that cardiorespiratory fitness keeps company with a structurally healthier brain, including the wiring that connects its regions, not just the grey matter that usually gets the headlines. It is informational rather than prescriptive: the associations are small at the global level and the studies mostly compared fitter people with less fit ones, so nobody can yet say that adding exercise rewires anything. Still, the direction of the evidence is consistent, and it adds brain wiring to the list of things that track with being fit.
