Why it matters

Cognitive decline is one of the most feared parts of ageing, and exercise is one of the few interventions with solid evidence behind it. A newer idea is to combine physical exercise with cognitive training in one programme, on the logic that challenging the brain while the body works may amplify the benefit. Yet it has not been clear whether the combination genuinely beats exercise on its own, or which type of exercise pairs best with mental training. This review set out to answer both questions in older adults.

The authors searched four databases without time restriction and pooled 24 studies involving 1079 older participants, with an average age in the early seventies, including people with and without mild cognitive impairment. Eligible studies compared a combined intervention, exercise plus cognitive training, against exercise alone. Outcomes were cognitive function and blood biomarkers, notably BDNF, a protein linked to brain plasticity. Results were combined using random-effects meta-analysis, and the quality of each study was rated with a modified checklist.

What they found

Combined interventions produced a small but statistically significant extra benefit to overall cognitive function compared with exercise alone, with an effect size of 0.335. The advantage held across exercise types: aerobic, resistance and multicomponent programmes all showed significant additional gains when cognitive training was added, with resistance-based combinations showing the largest of the three. Blood BDNF, however, showed no significant additional change, so the expected biological pathway behind the improvement is not confirmed here. The authors caution that limitations in the underlying studies mean the results should be interpreted carefully.

Where it fits

Exercise alone has repeatedly been linked to better cognition in later life, while cognitive training alone has shown mixed transfer to everyday thinking. This analysis suggests the two together add a little more than exercise by itself, which supports the dual-task and combined programmes already popular in ageing research. It complicates the mechanistic story, though, because the marker most often invoked, BDNF, did not move. Open questions include the best dose and sequencing of the two elements, how long the benefit lasts, and whether people with and without cognitive impairment respond differently.

What it means for you

If you already train partly for brain health, this is a reason to think that adding structured mental challenge alongside exercise could offer a small extra edge, whatever type of training you prefer. The improvement was modest, so the message is not that exercise alone is inadequate; it remains the foundation. It also suggests that tracking blood BDNF would not be a useful way to measure any benefit, since that marker did not change in these studies.
