Why it matters
Intermittent fasting has become one of the most popular strategies for managing weight and improving cardiometabolic health, yet most of the evidence treats adults as one homogeneous group. Bodies change with age: how muscle is preserved, how insulin behaves, how blood lipids respond. Whether a twenty-something and a sixty-something get the same benefits, or run the same risks, from the same fasting pattern has been unclear. This review set out to map the effects of intermittent fasting on body composition and cardiometabolic markers separately across adult age groups, and to surface any age-specific risks hiding inside the averages.

Following PRISMA guidelines, the researchers searched PubMed, Scopus and Web of Science for randomised controlled trials published up to September 2025, then stratified participants into three cohorts: under 30, 30 to 44, and 45 and over. They pooled 28 trials covering 1833 people, analysing body composition, lipid profiles, glycaemic markers and blood pressure. Subgroup analyses compared time-restricted feeding with intermittent energy restriction, and a deliberately conservative sensitivity analysis stress-tested the findings. Risk of bias and the certainty of the evidence were formally assessed.

What it means for you
If you fast for weight control, the weight result itself looks consistent across ages: it is the surrounding effects that deserve attention. This is a reason to think fasting is not automatically protective for cholesterol, and that lean tissue can go along with the fat, particularly for younger and older adults.
