Why it matters

Drinking has long been associated with higher blood pressure in population studies, but association is not the same as cause: heavy drinkers may differ from abstainers in plenty of ways that also raise blood pressure. The question that matters for anyone with creeping readings is whether cutting down on alcohol actually lowers the numbers. Randomised trials, where reducing alcohol is the only planned difference between groups, are the cleanest way to answer it. This analysis set out to pool every such trial and put a figure on what alcohol reduction does to blood pressure.

What they did

The researchers gathered randomised controlled trials published before June 1999 in which alcohol reduction was the only intervention difference between the active and control groups, ending up with 15 trials and 2234 participants in total. Three investigators independently extracted details on sample size, participant characteristics, study design, intervention methods, duration and results, following a standard protocol. Findings from the individual trials were then pooled, with each trial weighted by the precision of its result, to produce overall estimates for both systolic and diastolic blood pressure.

What they found

Alcohol reduction was associated with a significant fall in blood pressure: systolic dropped by an average of 3.31 mm Hg and diastolic by 2.04 mm Hg. A dose-response relationship emerged, meaning the greater the percentage reduction in alcohol intake, the greater the fall in blood pressure. The effect was also enhanced in people with higher blood pressure at the start, so those with the most to gain gained the most. On the strength of this, the authors recommended alcohol reduction as an important component of lifestyle change for preventing and treating high blood pressure among heavy drinkers.

Where it fits

This analysis took a well-known association from population studies and tested it with the strongest design available, pooled randomised trials, and the link held up as a genuine effect of drinking on blood pressure. A few mm Hg may sound small, but the dose-response pattern strengthens the causal case considerably. The trials were all published before June 1999, so the evidence base is not new, and the pooled figures speak most clearly about heavy drinkers rather than light or moderate ones. How long the reductions persist beyond the trial periods is a question this abstract does not address.

What it means for you

If you drink heavily and your blood pressure runs high, this is about as direct as trial evidence gets that cutting back will bring your readings down. The dose-response finding is worth sitting with: the bigger the reduction in drinking, the bigger the drop in pressure, so partial cuts still counted. For light drinkers with normal blood pressure the study has less to say, since the largest effects appeared in those starting from higher readings. Worth knowing that this sits alongside, not instead of, the other lifestyle levers that move blood pressure.
