What they did

The authors searched five major databases for studies published between January 2015 and 15 June 2026 linking Mediterranean dietary patterns, physical activity and related lifestyle factors to mortality or major adverse cardiovascular events in people already diagnosed with coronary heart disease. They found 40 reports drawing on 34 separate data sources and at least 260,354 unique patients. The mix included six randomised trials, 30 observational reports and four comparative nonrandomised studies. Comparable risk estimates were pooled statistically, risk of bias was assessed with design-appropriate tools, and the certainty of each finding was formally graded.

What they found

In the one randomised comparison of a Mediterranean diet against a low-fat diet, the Mediterranean pattern carried a hazard ratio of 0.72 for major adverse cardiovascular events, rated moderate certainty. Observational studies tied closer dietary adherence to lower all-cause mortality, with a pooled hazard ratio of 0.74, though its link to cardiovascular mortality specifically remained uncertain. For physical activity, people who maintained or continued being active had hazard ratios of 0.64 for all-cause mortality and 0.42 for cardiovascular mortality compared with those who stayed persistently inactive. Randomised evidence on exercise programmes, alone or combined with diet, was sparse.

Where it fits

The direction of these findings matches what is known about prevention in the general population and extends it into life after a coronary heart disease diagnosis. The important gap is that most of the evidence comes from watching patients rather than assigning them to a lifestyle, with only six randomised trials in the pool. Associations of this kind can be inflated because people who eat well and stay active often differ in other healthy ways too. The authors call for standardised, adequately powered trials, especially of exercise and of combined diet and activity programmes.

What it means for you

If you or someone close to you lives with coronary heart disease, this is a reason to think of diet quality and regular movement as still relevant after diagnosis, not just before it. The associations were sizeable, particularly for staying active, where cardiovascular deaths were markedly lower among those who kept moving. Because most of the data is from following people rather than testing them, it cannot promise that changing habits delivers those exact numbers.
