Why it matters

Physical inactivity drives a significant share of the world's cardiovascular disease burden, and sedentary behaviour is becoming more common, not less. Gym memberships and structured training work for some, but public health increasingly looks for movement that fits inside ordinary life: no kit, no cost, no scheduling. Stairs are everywhere, in workplaces, stations and homes, which makes stair climbing one of the most practical candidates for incidental exercise. This review asked whether choosing the stairs actually tracks with lower cardiovascular risk and mortality.

What they did

The authors systematically searched PubMed and Embase from inception until 8 April 2025 for randomised controlled trials or observational studies that included stair climbing as a variable. The outcomes of interest were cardiovascular mortality and the incidence of cardiovascular disease, including myocardial infarction, ischaemic stroke and heart failure. The search yielded 1873 studies, of which nine met the criteria, together covering 480,479 people. Analyses were run in the Review Manager software, and the review was registered on PROSPERO.

What they found

The headline result came from pooling five studies covering 455,619 people: stair climbing was associated with a significant reduction in cardiovascular mortality, a relative risk of 0.61 with a confidence range of 0.48 to 0.79. The same studies also linked stair climbing to lower all-cause mortality, a relative risk of 0.76 with a range of 0.62 to 0.94. Cardiovascular morbidity, illness rather than death, was also associated with stair climbing. The authors conclude that promoting everyday activities like taking the stairs, at work and at home, could help mitigate cardiovascular risk.

Where it fits

This finding sits comfortably within a broader shift in the physical activity literature, away from the idea that only formal exercise counts and towards evidence that incidental daily movement relates to longevity. The strength here is scale, nearly half a million people, but the weakness is design: because most included studies observed people rather than assigning them to climb stairs, healthier people may simply choose the stairs more often. The size of the association is striking enough to warrant proper intervention trials. How many flights, how fast and for whom the benefit is largest all remain open questions.

What it means for you

This is a reason to view the staircase as more than a nuisance between you and the lift: in large pooled data, the people who used it fared considerably better on cardiovascular and overall mortality. Because the evidence is associative, it cannot promise that switching to the stairs delivers those numbers, only that stair climbing keeps company with better outcomes. As everyday habits go, it is free, brief and repeatable many times a day. Treat it as one more data point in favour of building movement into ordinary routines.
