Before this review it was unclear whether interventions built around these devices actually improve day-to-day physical activity in people living with chronic diseases, and whether any extra movement translates into measurable health benefits under free-living conditions. That matters because people with conditions like type 2 diabetes and cardiovascular disease stand to gain the most from moving more, yet often find it hardest. The researchers set out to test whether tracker-based programmes deliver on both counts.

What they did

The team searched five bibliographic databases and, after removing duplicates from 8147 records, included 35 randomised controlled trials. Each trial compared a consumer wearable activity tracker-based counselling intervention against a control among patients with chronic respiratory diseases (7 trials), type 2 diabetes (12), cardiovascular diseases (6), overweight or obesity (3), cognitive disorders (1) or sedentary older adults (6). Results across trials were pooled using a random-effects model, with physical activity and cardiometabolic markers as the outcomes of interest.

What they found

Compared with control groups, tracker-based interventions significantly increased physical activity by 2123 steps per day. Health markers moved in the right direction too: systolic blood pressure fell by 3.79 mmHg, waist circumference by 0.99 cm and LDL cholesterol by 5.70 mg/dl. All of these changes were statistically significant. The individual improvements are modest in size, but they were achieved on top of usual care and under real-world, free-living conditions rather than in a supervised laboratory setting.

Where it fits

This review answered an open question about whether the tracker boom translates into genuine behaviour change in the people who most need it, and the answer was broadly yes. An important nuance is that these were tracker-based counselling interventions: the device was the anchor of a structured programme rather than a gadget handed over in isolation. That leaves open which components do the work, whether the device or the human support matters more, and how long the gains last once the programme ends.
