Why it matters

Exercise is universally recommended for people with overweight or obesity, but the guidance rarely settles the practical question: which kind? Endurance work, interval sessions, resistance training and various combinations all have advocates, and each targets slightly different aspects of health. For someone with limited hours in the week, knowing which modality moves the most markers matters. This analysis set out to rank five exercise types against each other on cardiometabolic health, something individual trials almost never do.

What they did

The researchers searched four databases up to September 2020 for randomised controlled trials of five exercise modalities: continuous endurance training, interval training, resistance training, combined aerobic and resistance training, and hybrid-type training. Outcomes spanned body measurements, body composition, blood lipids, glucose control, blood pressure, cardiorespiratory fitness and muscular strength. In total 81 studies contributed 4331 participants, 59% of them female, with a mean age of 38.7 years. A network meta-analysis then ranked the modalities, drawing on both direct and indirect comparisons.

Combined aerobic and resistance training emerged as the most effective modality for improving cardiometabolic health outcomes, with hybrid-type training second. Both multicomponent approaches outperformed the single-component options: continuous endurance training, interval training and resistance training alone. A subgroup analysis suggested the effects of different exercise types were mediated by gender, meaning men and women may not respond identically to the same programme. The overall message favoured breadth over specialisation.

The finding lines up with the latest exercise guidelines for people with overweight or obesity, which already lean towards multicomponent programmes, and gives those guidelines firmer quantitative footing. It complicates the popular framing of cardio versus weights as an either-or choice, since the winning approach was explicitly both. Open questions include the ideal balance between aerobic and resistance work within a combined programme, and why responses appeared to differ by gender.

What it means for you

If you carry extra weight and want the biggest return on your training time across blood pressure, blood sugar, lipids, fitness and strength, this is a reason to think a mixed programme outperforms picking a single lane. It does not say that any one modality is useless; all were studied precisely because each does something. The ranking simply suggests that combining them captures more of the benefits at once.
