Why it matters
Sarcopenic obesity is the combination of age-related muscle loss and excess body fat, and it leaves older adults doubly exposed: less strength to move a heavier body. Balance-related physical performance is what stands between an older person and a fall, and falls are one of the biggest threats to independence in later life. This review set out to establish whether standalone resistance training actually improves balance-related performance in this group, and whether older adults with sarcopenic obesity can stick with it safely.

The authors ran a systematic search of five databases up to 2 April 2026 for randomised controlled trials of standalone resistance training in adults aged 60 and over with sarcopenic obesity. Six trials met the criteria, covering 221 participants in total. Study quality was assessed with an established physiotherapy rating scale and the certainty of the evidence was graded, and the results were combined using pooled statistical models. The outcomes of interest were balance-related physical performance tests, principally the Timed Up and Go Test and the Short Physical Performance Battery, alongside adherence, dropout and adverse events.

Resistance training improved Timed Up and Go performance by 2.23 seconds on average, a test where lower times mean quicker, steadier movement. Short Physical Performance Battery scores rose by 0.93 points. Notably, the trials agreed with each other closely on both outcomes, which strengthens the pattern even though the trials were small. Adherence across the included trials ranged from 81.0 to 97.6%, dropout ranged from 6.7 to 27.3%, and no serious adverse events were reported. Where it fits
Resistance training already has a strong general case in older adults, but sarcopenic obesity is a distinct and harder population, and evidence specific to it has been sparse. This review extends the case to that group and adds the practical reassurance that people largely turned up and tolerated the training. The picture is far from complete: six trials and 221 people is a thin base, the certainty grades were low, and the review says nothing about which loads, exercises or programme lengths work best. Whether the test improvements translate into fewer real-world falls also remains an open question.

If you or someone in your family is older and carrying both low muscle and extra body fat, this is a reason to see resistance training as a realistic option rather than something reserved for the already fit. The improvements here were on tests that track everyday capability, standing up, walking and turning, which is what independence in later life actually rests on.
