Why it matters
Sarcopenia is the progressive loss of muscle mass and function that comes with age, and it erodes independence long before it looks like frailty. This review set out to test both ideas at once: what does more than 10 weeks of resistance training do to muscle structure, muscle function and bone mineral density in this specific group?
What they did
The researchers searched six major databases, including PubMed, Web of Science, Embase, MEDLINE, the Cochrane Library and CINAHL, for randomised controlled trials. To qualify, a trial had to run resistance training for more than 10 weeks in people over 70 with sarcopenia. Thirteen studies made the cut, covering 2,080 older adults in total. Results across the trials were pooled to estimate the effect of training on handgrip strength, isometric muscle strength, the chair stand test, skeletal muscle mass and bone mineral density.
What they found
Training worked for muscle on every pooled measure. Handgrip strength rose by an average of 1.67 kg compared with control groups, and isometric muscle strength improved as well. Performance on the chair stand test, a practical marker of leg strength and everyday function, was significantly better in the training groups, and skeletal muscle mass index also increased. The exception was bone: more than 10 weeks of resistance training produced no favourable effect on bone mineral density in this population.
Where it fits
The muscle findings confirm what smaller trials have long suggested: age, and even diagnosed sarcopenia, do not switch off the body's ability to respond to resistance training. The bone result is the more useful correction, because lifting is often presented as a two-for-one deal for muscle and skeleton alike. In this age group and over this timeframe, the second benefit did not materialise. Open questions remain about whether longer programmes, heavier loading or combined approaches could move bone density where these trials did not.
What it means for you
If you are past 70, or helping a parent who is, this is a reason to think resistance training remains one of the most valuable things an older body can do: stronger grip, more muscle and an easier time getting out of a chair are exactly the outcomes that keep people independent. It is also a reason to keep expectations honest about bone. On this evidence, a lifting programme of this length is a muscle intervention rather than a bone one.
