Why it matters

Sarcopenia and osteoporosis, the age-related loss of muscle and bone, drive frailty, functional decline and fractures. The authors conducted a systematic review and meta-analysis of randomised controlled trials comparing high-intensity resistance training, defined as 70% of one-repetition maximum or above, against low-to-moderate-intensity training below that threshold, in adults aged 50 and over. Eighteen studies covering 1283 participants were included. Primary outcomes were lower limb strength measured by leg press and leg extension, plus bone mineral density at the lumbar spine and femoral neck. Fall incidence and adverse events were tracked as secondary outcomes.

What they found

High-intensity training clearly won on strength: the pooled advantage was large for leg press (standardised mean difference 0.95) and moderate for leg extension (0.63).
