Lifting weights cools one inflammation marker in over-60s, but not the others

Why it matters

Ageing tends to come with a low, persistent level of inflammation in the body, and that background inflammation is linked with conditions such as sarcopenia, the age-related loss of muscle. That loss in turn erodes functional capacity: the everyday ability to walk, climb stairs and get out of a chair. Resistance training is already regarded as one of the most useful interventions for older adults, but whether it also dampens inflammatory markers, and which markers respond, has been less clear. This review set out to answer that question in healthy people aged 60 and over, rather than in patients with a diagnosed disease.

What they did

The authors systematically searched six databases, including Medline, PubMed, Web of Science, Cochrane Library and Embase, for randomised controlled trials published up to December 2023. Eligible trials tested resistance training in healthy adults aged 60 years and over and measured inflammatory markers, body composition or functional capacity. The markers of interest were C-reactive protein, interleukin-6 and tumour necrosis factor alpha, alongside body weight, BMI, fat percentage, fat mass, lean mass, leg extension strength and the six-minute walk test. Nineteen trials involving 728 participants met the criteria, and the results were pooled, with a meta-regression planned to explore how training variables might moderate the inflammatory findings.

What they found

C-reactive protein fell significantly after resistance training programmes, with a mean difference of 0.74 and a p-value of 0.008. The other two inflammatory markers told a different story: TNF-α showed essentially no change, with a mean difference of 0.1 and a p-value of 0.95, and IL-6 did not shift significantly either, with a mean difference of 0.27 and a p-value of 0.12. Beyond the blood markers, resistance training improved leg extension strength and six-minute walk performance, which are two of the most practical measures of how well an older body actually works.

Where it fits

This pooled analysis strengthens the case that resistance training does more for older adults than build muscle: it also nudges at least one systemic inflammation marker in the right direction. At the same time, it complicates the simple story that lifting is broadly anti-inflammatory, since TNF-α and IL-6 did not respond. The authors themselves call for further research to reach a clear conclusion and to understand the mechanisms behind these effects. Open questions include why CRP responds when the other markers do not, and which combinations of training load, frequency and duration matter most.

What it means for you

For a healthy person over 60, this is a reason to see resistance training as more than a muscle-building habit: across 728 trial participants it lowered C-reactive protein, a widely used marker of background inflammation, and reliably improved leg strength and walking capacity. It is equally a reason to keep expectations honest, because two other inflammation markers did not move. The functional gains are arguably the headline for daily life, since leg strength and walking endurance are closely tied to staying independent. Anyone hoping lifting alone will rewrite their entire inflammatory profile will not find support for that here.
