Why it matters

Resistance training is routinely recommended to women for improving body composition, yet women are not one homogeneous group. Menopause changes hormones and how fat is carried, excess weight changes the starting point, and activity history changes how much room there is to improve. Whether lifting produces the same fat and muscle changes across these different groups had not been clearly mapped. This review asked whether resistance training alone affects fat mass, body fat percentage, fat-free mass and muscle mass differently depending on which group of women is doing it.

What they did

The authors searched the PubMed and Web of Science databases, with two reviewers screening studies simultaneously, and included 23 randomised controlled trials of resistance training in women. Study quality was assessed with the Cochrane risk of bias tool. They extracted sample sizes, means and standard deviations for fat mass, body fat percentage, fat-free mass and muscle mass from the original articles, then compared pooled effects across subgroups: postmenopausal versus premenopausal women, overweight or obese versus not, and sedentary versus non-sedentary women.

What they found

The clearest responders were postmenopausal women, in whom resistance training significantly reduced body fat percentage and fat mass while increasing fat-free mass. Premenopausal women saw only a clear reduction in body fat percentage. Overweight or obese women lost fat mass and body fat percentage and gained fat-free mass, whereas women who were not overweight showed no significant body composition change. Sedentary women improved across fat mass, body fat percentage and fat-free mass, while already-active women gained only fat-free mass.

Where it fits

These results extend the general case for resistance training by showing that the size of its body composition effect depends on who starts it. That fits an intuitive pattern seen elsewhere in exercise science: people furthest from their potential have the most measurable room to improve. The review's subgroup approach cannot say how large each difference is for an individual, and the included trials varied in their programmes. It also leaves open whether lean, trained women gain other benefits that body composition measures simply fail to capture.

What it means for you

If you are a woman who is postmenopausal, sedentary or carrying extra weight, this pooled evidence suggests resistance training is likely to shift your body composition measurably, cutting fat while adding fat-free mass. If you are already lean and active, smaller changes on these particular measures are the realistic expectation, which is not the same as lifting being pointless. It is a useful corrective to the idea that visible change comes equally easily to everyone. Reading your own results against your own starting point is the fairer comparison.
