Why it matters
Menopause is associated with adverse changes in body composition, and physical activity is widely proposed as a way to counter them. What has been far less clear is how large and how consistent that effect actually is, given that exercise programmes vary enormously in frequency, intensity, type and duration. There is also a second question that gets much less attention: whether exercise changes not just the body itself but how women perceive their bodies. This review set out to pull together the trial evidence on both outcomes at once, which are usually studied separately.
What they did
The researchers searched MEDLINE, Web of Science and Scopus for randomised controlled trials published in English between 2014 and 2025 that tested structured physical activity programmes in postmenopausal women and reported body composition or body image outcomes. After duplicates were removed, 2,483 records were screened and 13 trials involving 694 women met the inclusion criteria. Risk of bias was assessed with a standard tool and the certainty of the evidence was formally graded. Pooled differences were then calculated for body mass index, percentage of fat mass and waist-to-hip ratio using a random-effects model.

Structured physical activity was linked to significant reductions in BMI of 0.87 kg/m2, in percentage of fat mass, and in waist-to-hip ratio of 0.03. The BMI and waist-to-hip ratio results were remarkably consistent across trials, while the fat mass findings varied moderately from study to study. Most included trials were judged to be at low risk of bias or to raise only some concerns.
