Why it matters

Raised blood pressure is one of the most consequential risk factors for heart disease, and diet is one of the few levers everyone holds. Dietary fibre has long been recommended for cardiovascular health, but fibre is not one substance: it comes in many forms, from the gel-forming fibres in oats to the roughage in wholegrains. Whether every kind of fibre earns its blood pressure reputation, or only certain types do, was the question this review set out to answer in healthy people rather than patients. Untangling the types matters because blanket advice can bury the foods doing the real work.

What they did

The reviewers searched the literature for randomised controlled trials published between 1 January 1990 and 1 December 2013 that tested a fibre isolate or a fibre-rich diet against a control or placebo for at least 6 weeks. Twenty-eight trials reported fibre intake alongside systolic or diastolic blood pressure, and 18 supplied enough data to be pooled. Each study was sorted into 1 of 12 fibre-type categories so that different fibres could be judged separately rather than lumped together. Random-effects models were used to combine the results.

What they found

Pooled across every fibre type, the effect was underwhelming: systolic pressure fell by 0.9 mmHg and diastolic by 0.7 mmHg, and neither change was clearly distinguishable from no effect at all, for a median difference in total fibre of 6 g. Beta-glucans were the exception. Diets rich in this oat and barley fibre cut systolic pressure by 2.9 mmHg and diastolic by 1.5 mmHg for a median difference of just 4 g. Results for individual fibre types were generally consistent with one another, which strengthens the beta-glucan signal.

Where it fits

The findings sit comfortably with existing advice to eat more fibre-rich foods, but they sharpen it: the blood pressure benefit appears to concentrate in beta-glucan sources such as oats and barley rather than in fibre as a whole. That distinction is easy to lose in blanket fibre recommendations. Open questions remain, including whether the effect holds over longer periods, whether people with existing high blood pressure would see larger changes, and exactly how beta-glucans exert their effect on pressure.
