Why it matters

Regular movement is central to living well with diabetes, yet long stretches of sitting dominate modern life. Technological advances and easy internet access have produced a flood of digital behaviour change interventions, from apps to online coaching, all promising cheap and scalable nudges towards activity. The open question is whether these tools actually change what people with diabetes do day to day, rather than being downloaded and forgotten. This review set out to weigh the trial evidence on both sides of the ledger: raising physical activity and cutting sedentary time.

The researchers ran a comprehensive search of seven databases, including a dedicated sedentary behaviour research database, looking for randomised controlled trials of digital behaviour change interventions in adults with diabetes. Two reviewers independently handled study selection, data extraction, risk of bias assessment and evidence quality evaluation. Thirteen trials with 980 participants met the inclusion criteria. Where the data allowed, results were pooled in meta-analyses; otherwise the findings were summarised narratively, covering steps, sedentary time and its breaks, light activity and moderate-to-vigorous activity.

Overall, the digital interventions significantly increased daily steps and the number of breaks people took from sitting. Subgroup analyses added texture: interventions using more than 10 behaviour change techniques improved steps, light activity and moderate-to-vigorous activity. Step counts also rose in programmes of moderate and long duration, in those drawing on more than 4 clusters of behaviour change techniques, and in those combined with a face-to-face component. Studies using at least 2 digital components improved steps, light and moderate-to-vigorous activity, and reduced sedentary time.

Where it fits

This pooling supports the idea that digital tools can shift real-world movement in people with diabetes, and it suggests that richer, multi-component programmes outperform simple ones. The authors are careful, though: they describe the evidence as 'some', and call for more high-quality studies before firmer conclusions can be drawn. Almost everything here concerns type 2 diabetes, and the potential of digital interventions for adults with type 1 diabetes remains an explicitly open question the authors flag for future work.

What it means for you

For a reader managing type 2 diabetes, or simply interested in how technology changes behaviour, this is a reason to take digital activity tools seriously rather than dismissing them as gimmicks.
