Why it matters

Chronic low back pain is the most frequent musculoskeletal disorder, and it erodes both comfort and the ability to manage daily life. Women carry a substantial share of that burden, and postmenopausal women in particular are a group whose response to exercise treatments has been under-examined. Pilates is widely offered for back pain, but whether it reliably helps women specifically, and whether the benefit survives the menopause, needed a systematic answer. This review set out to pool the randomised evidence on exactly those questions.

What they did

The reviewers searched six databases, PubMed, Embase, Web of Science, the Cochrane Library, PEDro and CINAHL, up to 11 April 2026 for randomised controlled trials comparing Pilates with no intervention or with other interventions in women with chronic low back pain. Results for pain and function were pooled statistically, with subgroup analyses by age, follow-up time and region, and further subgroups among postmenopausal women. Study quality was assessed with an established tool, and the authors checked whether any single study drove the findings and whether publication bias was likely.

What they found

Pilates was associated with substantial pain relief, an effect of 1.88 on the standardised scale researchers use, which counts as very large, alongside clear functional improvement at 1.08. Postmenopausal women improved on both counts, with effects of 2.07 for pain and 1.11 for function, while non-menopausal women also benefited, at 1.58 for pain and 1.03 for function. All of these results were statistically significant. In short, the benefit did not depend on menopausal status.

Where it fits

Exercise is a mainstay of chronic low back pain management, and this analysis adds weight to Pilates specifically as an option for women. The postmenopausal subgroup result is the most novel contribution, since older women are often assumed to respond less well to exercise interventions. Some caution is warranted: effects this large are unusual in back pain research, and the comparison groups ranged from no intervention to other treatments, so the pooled numbers may overstate what Pilates adds beyond structured exercise in general. Head-to-head comparisons against other exercise types remain an open question.

What it means for you

If you are a woman living with long-standing back pain, this is a reason to consider Pilates a credible, evidence-backed option, and menopause appears to be no barrier to benefiting. The sheer size of the pooled effect deserves some scepticism, but the direction of the finding is consistent across groups. What this analysis cannot tell you is whether Pilates would beat another form of exercise you already do and enjoy.
