Why it matters
Adults with type 2 diabetes commonly lose muscle strength and physical function over time, and that decline makes the disease harder to manage in the long run. Resistance training is a standard recommendation, but gym machines and free weights are not accessible or affordable for everyone. Elastic bands are inexpensive, portable and usable at home, which makes them an appealing alternative, yet the evidence on whether band training actually improves blood sugar control, strength, physical function and muscle mass had never been comprehensively pulled together. The authors searched five databases, including PubMed, Web of Science and three major Chinese research databases, from their inception to June 5, 2026, for randomised controlled trials of elastic-band resistance training in adults with type 2 diabetes. Fourteen trials involving 684 participants qualified. Effects were pooled as mean differences or standardised mean differences, and the certainty of each finding was formally graded. The reviewers also ran sensitivity checks, removing one trial at a time to see whether any single study was driving the overall results.
What they found
Band training was associated with a reduction in HbA1c of 0.41 and in fasting plasma glucose of 0.79, both rated moderate-certainty, and both results held when individual trials were left out. Handgrip strength improved by 2.67, with the included trials in complete agreement on the direction of the effect. Muscle mass showed a small favourable signal of 0.24, but the estimate brushed against no effect and the certainty was low. Walking ability and sit-to-stand performance trended favourably, yet the trials disagreed with each other too much for confidence, and adverse events were reported too inconsistently to judge safety.
Where it fits
Resistance training already has an established place in exercise guidance for type 2 diabetes, but this is a comprehensive synthesis focused specifically on elastic bands, the cheapest and most portable form of the training. The glycaemic findings extend what was known from equipment-based training to a genuinely home-friendly format. The uncertain results for muscle mass and physical function, along with the patchy reporting of side effects, are the gaps the authors highlight, and they call for further high-quality trials to pin down the ideal way to prescribe band training.
What it means for you
If distance, cost or confidence keeps you out of the gym, this is a reason to think a set of elastic bands could still deliver something meaningful for blood sugar control and grip strength in type 2 diabetes. The evidence is moderate rather than definitive, the best routine has not been established, and the review cannot say much about safety because side effects were poorly tracked across the trials.
