Why it matters
Dietary nitrate is one of the most studied performance supplements in existence, with over 100 studies and reviews examining its effects in young, healthy men and women. What has been far less clear is whether the well-described benefit applies equally to everyone. Biological sex, aerobic fitness and the oxygen content of the air breathed during exercise are all plausible modifiers, and if the effect fades in certain groups, that changes who should care about the supplement at all. This meta-analysis set out to size the overall effect honestly and to test those three factors directly.
What they did
The researchers systematically searched for randomised, crossover, placebo-controlled studies reporting exercise performance outcomes with nitrate supplementation in young, healthy adults. From 2033 articles identified, 80 met the inclusion criteria and entered a random-effects meta-analysis. Beyond calculating the overall pooled effect across different exercise paradigms and supplementation approaches, subgroup analyses examined biological sex, aerobic fitness level, and the fraction of inspired oxygen during exercise, comparing hypoxic conditions with normal air.
What they found
Overall, nitrate supplementation improved exercise performance compared with placebo, but the effect was objectively small (d = 0.174). Three subgroup results stand out. The benefit was not observed in the six studies enrolling only women. It was also absent in well-trained endurance athletes, defined by aerobic fitness of at least 65 mL per kg per minute, across 26 studies. The oxygen content of inspired air made no difference, with similar effects in hypoxia and normoxia. The clear ergogenic effect was therefore concentrated in recreationally active, young, healthy men.
Where it fits
This pooling confirms that nitrate's performance effect is real while putting an honest size on it, and it sharpens the boundaries: highly trained endurance athletes, who have already optimised other training parameters, appear to gain little or nothing. The null finding in women rests on just six studies, so it may reflect a shortage of data rather than a true sex difference. The authors call for mechanistic work and for studies in a wider variety of participants, arguing that yet more descriptive studies of young healthy men would add little to what is already known.
What it means for you
If you are recreationally active and curious about nitrate, the evidence says a performance boost exists but is small, so expectations should be sized accordingly. If you are a well-trained endurance athlete, the pooled data give little reason to expect any benefit at all. For women, the honest answer is that the evidence is too thin to say either way, which is itself worth knowing when supplement marketing implies the effect is universal.
