Pooling all the trials is the only fair way to decide whether the compound does anything, and if so, how much. This meta-analysis set out to answer exactly that for human endurance exercise capacity.

The authors searched the literature using the key words quercetin, performance, exercise, endurance and aerobic capacity, and identified 11 studies meeting their inclusion criteria, providing data on 254 human subjects. Across the studies, participants' baseline VO2max ranged from 41 to 64 mL per kg per minute with a median of 46, so the pool spanned recreationally fit through well-trained people. The median treatment duration was 11 days and the median dose was 1000 mg per day. Effect sizes were calculated as standardised mean differences and pooled with a random-effects model.

What they found

Combining VO2max and endurance performance measures, quercetin showed a statistically significant benefit over placebo, with an effect size of 0.15, equating to roughly a 2% improvement. The authors class that magnitude as between trivial and small. A subgroup analysis found no significant difference between quercetin's effect on endurance performance versus its effect on VO2max. Meta-regression showed the effect did not vary with participants' fitness level or with the plasma quercetin concentration achieved, so higher blood levels did not buy a bigger benefit.

Where it fits

This review resolves the mixed individual findings in a way that satisfies neither camp: the sceptics were wrong that quercetin does nothing, and the enthusiasts were wrong that it matters. A statistically significant effect that equates to about 2% sits well below what most athletes would notice, and far below what established ergogenic aids deliver. The finding that neither fitness level nor plasma concentration explained the variation between studies leaves open what does, and the authors note those experimental factors remain to be elucidated.

What it means for you

This is a textbook example of the difference between statistically significant and practically meaningful. Quercetin genuinely moves endurance capacity, but by an amount, roughly 2%, that would be hard to distinguish from a good night's sleep or a strong warm-up. For anyone deciding where to spend supplement money for endurance, this is a reason to look elsewhere, since even bigger doses achieving higher blood levels did not produce bigger effects. The interesting takeaway is less about quercetin and more about reading supplement claims: always ask how large the effect is, not just whether one exists.
