Why it matters

Standard cholesterol panels tell only part of the cardiovascular story. Apolipoproteins, lipid and apolipoprotein ratios, and lipoprotein sub-fractions may predict cardiovascular disease risk more effectively than the familiar totals, yet whether aerobic exercise moves these deeper markers had not been established. Aerobic training is already prescribed as lipid management and is known to improve the standard profile, so knowing whether it also improves these newer biomarkers matters for how its heart benefits are understood and measured.

What they did

The reviewers searched major databases from inception to the end of December 2021 for randomised controlled trials in adults. To qualify, trials needed at least 10 participants per group, an aerobic exercise intervention lasting at least 12 weeks at more than 40% of maximum oxygen consumption, and measurements before and after. People who were already active, pregnant or lactating, or living with chronic disease beyond metabolic syndrome factors were excluded, as were trials testing diets, medications or resistance training. Fifty-seven trials totalling 3194 participants were analysed using multivariate meta-analysis.

What they found

Aerobic training significantly raised protective, antiatherogenic apolipoproteins and lipoprotein sub-fractions, by a mean difference of 0.047 mmol/L. It lowered the atherogenic, artery-harming apolipoproteins and sub-fractions, and improved atherogenic lipid ratios by a mean difference of 0.201. The absolute changes were small, but consistent in direction across the pooled trials. Meta-regression further suggested that features of the intervention itself, such as how the training was structured, contributed to how much these lipid, sub-fraction and apolipoprotein ratios changed.

Where it fits

This extends what was already established about aerobic exercise and the standard lipid profile into newer territory: the apolipoproteins and lipoprotein sub-fractions that may track cardiovascular risk more closely. It supports the idea that the heart benefits of aerobic training run deeper than a routine cholesterol test can show. The unproven step is the final one, since the trials measured biomarkers rather than heart attacks or deaths, so whether these marker improvements translate into fewer cardiovascular events remains an open question.

What it means for you

If your cholesterol numbers look fine but you want a fuller picture of cardiovascular risk, this is a reason to think regular aerobic exercise works on layers of blood lipids that standard tests never report. In previously sedentary adults, twelve or more weeks of at least moderate-intensity training shifted these deeper markers in a protective direction. The absolute changes were modest, and the finding describes risk markers rather than proven reductions in heart disease itself.
