Diet beat exercise for lowering LDL; training only shifted triglycerides

Why it matters

Diet and aerobic exercise are the two lifestyle levers people are told to pull when blood lipids look unfavourable, and they are frequently presented as interchangeable. Previous studies had reached conflicting conclusions about which one actually shifts cholesterol and triglycerides, and by how much. Very few trials compare both approaches, and their combination, against a control group within the same study, which is what you need to rank them fairly. Pooling those rare four-arm trials was the point of this analysis.

What they did

The authors searched nine electronic databases, cross-referenced and consulted expert review to find randomised controlled trials lasting at least 4 weeks in adults aged 18 or over. Eligible trials had to include all four arms: a diet group, an aerobic exercise group, a combined group and a control group. Outcomes were total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides. Two authors extracted data independently, and results were combined using random-effects models, with mixed-effects models used to compare the groups against one another. Six studies including 788 men and women met the criteria.

What they found

Diet alone and diet combined with exercise both produced clear reductions in total cholesterol, LDL cholesterol and triglycerides. Aerobic exercise on its own showed a clear reduction only in triglycerides. When the arms were compared directly, the reductions in total cholesterol and LDL cholesterol were greater for diet and for diet plus exercise than for exercise alone. HDL cholesterol did not change significantly in any group, which is a notable null given how often exercise is credited with raising it. The authors concluded that diet, and especially diet with exercise, is superior to exercise alone for these measures.

Where it fits

This sits awkwardly against the common assumption that cardio is the main lever for cholesterol, and it puts food first for the cholesterol-carrying particles. The triglyceride finding is consistent with exercise having a genuine but narrower lipid effect. The absence of any HDL change across all arms complicates a widely repeated claim, though 4 week trials may be too short for that marker. With only six trials, the ranking is best treated as a strong pointer rather than a final verdict, and nothing here speaks to hard outcomes such as heart attacks.

What it means for you

If your reason for training is a cholesterol number on a blood test, this is a reason to expect the bigger movement to come from changes to what you eat. It is not an argument against aerobic exercise, which lowered triglycerides here and carries plenty of benefits these trials did not measure. The combination performed as well as diet alone on cholesterol, so the two are not in competition. Do not expect either to lift HDL cholesterol on the basis of this evidence.
