Why it matters

Avocados have previously been shown to improve standard blood lipid profiles, but a standard cholesterol panel only tells part of the story. Cardiovascular risk research increasingly focuses on lipoprotein particles — the actual carriers of cholesterol in the blood — because the number and type of particles can matter beyond the cholesterol total. Whether a simple dietary addition like a daily avocado changes these particle-level measures has been unclear. This study set out to answer exactly that: does incorporating one avocado a day shift the concentrations of lipoprotein particles and their subclasses in people with abdominal obesity, a group at elevated cardiometabolic risk?

What they did

This was an ancillary study of the Habitual Diet and Avocado Trial, a multicentre, parallel-arm, 26-week randomised controlled trial in free-living adults with abdominal obesity. One group was provided with 1 avocado per day to add to their diet, while the comparison group was instructed to simply continue their habitual diet without supplemental avocados. Fasting plasma samples taken at baseline and at 26 weeks were analysed by nuclear magnetic resonance spectroscopy to measure lipoprotein particles and subclasses, which were exploratory outcomes. The analytical sample included 786 participants — 74% female, average age 51, average body mass index 32.9 — split between 389 in the avocado group and 397 in the habitual diet group.

What they found

The avocado group showed a reduction in total LDL particle concentration compared with the habitual diet group, with a mean difference of -49.1 nmol/L (95% CI -83.6 to -14.5, P = .005) over the 26 weeks. That was the extent of the effect: no between-group differences were observed for LDL particle size or LDL subclasses, for triglyceride-rich lipoprotein particles, for high-density lipoprotein particles, or for apolipoproteins A or B. In other words, the daily avocado moved one atherogenic particle measure while leaving the rest of the lipoprotein landscape unchanged. The authors conclude the intervention reduced atherogenic LDL particle concentration without affecting other particles and subclasses.

Where it fits

The finding extends earlier evidence that avocado intake improves blood lipids by adding particle-level detail from a large randomised trial rather than a small feeding study.
