Why it matters

Depression after childbirth affects a substantial number of new mothers, and it shapes not just their own well-being but early family life. Physical activity is cheap, widely accessible and free of the concerns some women have about taking medication while pregnant or breastfeeding, yet the evidence on whether it actually prevents or treats postpartum depression has been inconsistent. This meta-analysis set out to settle whether exercise during pregnancy and the postpartum period genuinely reduces depressive symptoms after birth, and whether it works differently for women who are already struggling compared with those who are not.

What they did

The researchers systematically searched seven databases, including Cochrane Library Plus, PubMed, EMBASE, CINAHL, Web of Science and Scopus, covering studies published from January 1990 to May 2016. They included randomised and nonrandomised controlled trials testing physical activity interventions against postpartum depression, pooling the results with an inverse variance-weighted method. Twelve studies met the criteria. The team then ran subgroup analyses splitting mothers by whether they met criteria for postpartum depressive symptoms at baseline on specific symptom scales, and used meta-regression and sensitivity analyses to probe how much the studies disagreed.

What they found

Overall, exercise during pregnancy and the postpartum period reduced postpartum depressive symptoms with a pooled effect size of 0.41, a moderate benefit, and the studies were reasonably consistent with each other. The subgroup analysis was the most telling part: for mothers who met depressive symptom criteria at baseline, the effect size rose to 0.67, while for mothers without baseline symptoms it was 0.29, smaller but still significant. In other words, exercise appeared to work both as treatment and as prevention. The authors conclude that physical exercise across this period is a safe strategy for better psychological well-being.

Where it fits

Previous evidence on exercise and postnatal mood had pointed in different directions, and this pooling helps resolve that inconsistency in favour of a real benefit. The finding that women with existing symptoms improved most fits the broader literature on exercise and depression in general adult populations. Important caveats remain: the review admitted nonrandomised trials alongside randomised ones, which can inflate effects, and the analysis cannot say which type, dose or timing of exercise works best. Whether the benefit persists long after the intervention ends is also not addressed here.

What it means for you

For women who are pregnant or recently gave birth, this is a reason to see physical activity as more than fitness upkeep: across a dozen trials it measurably lifted mood in the vulnerable months after delivery. The signal was strongest for mothers already experiencing depressive symptoms, which suggests movement can sit alongside, not instead of, proper care. It is worth being clear about what this is not: exercise is not presented here as a replacement for professional treatment, and anyone with significant symptoms deserves clinical support. The encouraging point is that something accessible appears to genuinely help.
