Why it matters

The researchers ran a meta-analysis, searching databases up to July 20, 2022 for randomised controlled trials of exercise interventions for perinatal depression. Forty-eight trials met their criteria, covering a total of 5282 pregnant women. They grouped the evidence four ways: exercise to prevent prenatal depression, exercise to treat prenatal depression, exercise to prevent postpartum depression, and exercise to treat postpartum depression. Within each grouping they compared modalities including yoga, aerobic exercise, combined aerobic and resistance work, gymnastics, pelvic floor muscle training, water exercise, fertility dance and stroller walking.
What they found
The headline pattern was that exercise worked better as treatment than as prevention, and better in the prenatal period than postpartum, with a moderate effect overall. For preventing prenatal depression the effect was low, with yoga ranking highest. For treating prenatal depression the intervention effect was significant, led by gymnastics, then pelvic floor muscle training, aerobic exercise, combined aerobic and resistance training, and yoga. Prevention of postpartum depression again showed only a low effect. Treating postpartum depression showed a moderate effect, led by aerobic exercise, then water exercise, yoga, fertility dance and stroller walking.
