Why it matters
Sleep disturbance is one of the most common complaints of menopause, and it is not a trivial one: it erodes quality of life and could raise the risk of other menopause-related diseases. Hormone-driven sleep problems are also notoriously difficult to treat, which makes low-cost, low-risk options worth testing properly. Exercise is the obvious candidate, but individual trials in menopausal women have been scattered across different exercise types and outcome measures. This review set out to synthesise the randomised evidence on whether exercise actually improves sleep in this group.
What they did
The authors ran a comprehensive search across seven electronic databases on 3 June 2022, looking for randomised controlled trials of exercise interventions in menopausal women. Seventeen trials met the criteria for the systematic review, and ten of them provided data suitable for statistical pooling. Outcomes included insomnia severity, sleep problems and sleep quality, and the quality of the included trials was assessed with a standard bias checklist. Effects were expressed as differences between the exercise and control groups.
What they found
Exercise significantly reduced insomnia severity, the largest and clearest effect in the analysis, and also alleviated sleep problems. Sleep quality told a different story: the difference between exercise and control groups was not statistically significant. A subgroup analysis found the effects of exercise were more apparent in women with sleep disorders than in women without them. The review could not determine which intervention duration was most beneficial, and the primary studies overall carried a moderate risk of bias.
Where it fits
This supports the general case that exercise helps sleep, while adding useful precision for menopause specifically: the benefit concentrates on insomnia rather than lifting every sleep measure, and it concentrates in the women whose sleep is genuinely disordered. The authors conclude exercise can be recommended to menopausal women for sleep, but they also call for better trials comparing different types of exercise, walking, yoga and meditative movement among them, over different durations, with both subjective and objective sleep measurement. Until then, the how and how much remain unsettled.
What it means for you
For women whose sleep has deteriorated through menopause, this is a reason to think regular exercise is associated with genuine relief, particularly if insomnia is the specific problem. The pattern in the subgroup analysis suggests the worse the starting point, the more there is to gain, whereas women sleeping reasonably well saw less change. The null result on general sleep quality is worth holding onto: exercise looked like a treatment for disturbed sleep, not a universal enhancer of good sleep. Which exercise, and for how long, the evidence cannot yet say.
