Why it matters

Fourteen trials covering 1201 adults met the inclusion criteria. Results were combined using a three-level statistical model, and study quality was rated with the PEDro scale. The team also ran subgroup analyses to test whether exercise type, intensity, weekly frequency, session length and programme duration changed the outcome.

Exercise produced a small but significant improvement in overall executive function, and in the subdomains of working memory and cognitive flexibility. In the subgroup analyses, significant gains appeared only for aerobic exercise, at moderate to vigorous intensity, performed three or more times per week, in sessions of 60 minutes or less, within programmes lasting at least 13 weeks. It sits comfortably alongside evidence in other adult populations that aerobic training supports cognition. Open questions remain about whether these executive gains translate into relief of depressive symptoms themselves, and whether other training styles could work if tested properly. The authors call for further randomised trials to pin down the therapeutic effects.

For anyone living with depression, this is a reason to think of regular aerobic exercise as support for mental sharpness, not just mood. The pattern in the data favoured consistency over heroic single sessions: several sessions of an hour or less each week, sustained over months, was where the benefits showed. The overall effect was small, so exercise is unlikely to transform thinking on its own. It sits alongside, rather than replaces, standard care for depression.
