Why it matters
Chronic inflammation and disturbed metabolic health sit at the heart of obesity's long-term risks, so improving both matters for prevention and treatment. Exercise is the obvious candidate, but how different training styles interact with inflammatory and cardiometabolic biomarkers is not completely understood. Individual trials have tested aerobic work, resistance work and other modalities with mixed results. This review set out to pool that scattered evidence and ask which markers exercise reliably moves in adults with overweight or obesity, and which it does not.

The authors systematically searched PubMed, CINAHL, MEDLINE, Cochrane, Google Scholar, Scopus and Web of Science up to December 2021. Thirty-five randomised controlled trials covering 2752 overweight or obese adults met the inclusion criteria. Results were combined using a random-effects model to estimate weighted mean differences, and subgroup analysis compared training types against each other. Outcomes spanned inflammatory markers such as C-reactive protein, interleukin-6 and tumour necrosis factor-α alongside cardiometabolic measures including insulin, glucose, triglycerides and maximal oxygen consumption.

Exercise training significantly increased adiponectin, a hormone linked to metabolic health, and raised maximal oxygen consumption by 5.26 mL/kg/min on average. It also reduced fasting insulin, fasting blood glucose, insulin resistance and triglycerides, with the triglyceride drop averaging 10.88 mg/dL. The inflammation picture was flatter: interleukin-6, tumour necrosis factor-α, leptin, C-reactive protein and cholesterol concentrations showed no significant changes. Subgroup analysis suggested aerobic and resistance training produced the most consistent benefits compared with other modalities.
Where it fits
The metabolic findings line up with a large body of work showing exercise improves insulin handling and blood lipids in people carrying excess weight. The inflammation result is more surprising: exercise is widely described as anti-inflammatory, yet the best-known markers did not budge across these pooled trials, with adiponectin the exception. That gap may reflect trial length, training dose or the markers chosen, questions this analysis cannot settle. The authors call for further long-term studies to confirm and expand the findings.
What it means for you
If your goal in training is metabolic health, this evidence is encouraging: pooled across dozens of trials, exercise moved insulin, glucose, triglycerides and fitness in the right direction for adults with overweight or obesity. It is also a reason to be sceptical of quick claims that a training block will slash your inflammation markers, since CRP and IL-6 stayed put here. Aerobic and resistance work showed the most consistent effects, which suggests neither style has a monopoly on the benefits.
