Why it matters
High blood pressure is one of the most consequential and most modifiable risk factors in medicine, and exercise is a first-line recommendation for preventing and treating it. But exercise comes in many flavours, from aerobic work and resistance training to high-intensity intervals and isometric holds, and guidance has historically leaned hardest on the aerobic evidence. What they did
The authors systematically searched PubMed, the Cochrane library and Web of Science for randomised controlled trials published between 1990 and February 2023, requiring an exercise intervention of at least 2 weeks and a non-intervention control group. That yielded 270 trials with a pooled sample of 15,827 participants. Pairwise analyses estimated each mode's effect on resting systolic and diastolic blood pressure, while a network meta-analysis ranked the modes against one another. Secondary analyses drilled down into specific submodes such as the isometric wall squat and running.
What they found
Every mode worked. Aerobic training lowered blood pressure by 4.49/2.53 mm Hg, dynamic resistance training by 4.55/3.04, combined training by 6.04/2.54 and high-intensity interval training by 4.08/2.50. Isometric exercise training led the field with reductions of 8.24/4.00 mm Hg. In the network rankings for systolic pressure, isometric training scored 98.3%, ahead of combined training at 75.7%, dynamic resistance at 46.1%, aerobic training at 40.5% and intervals at 39.4%, while the wall squat and running emerged as the most effective single exercises for systolic and diastolic pressure respectively.
Where it fits
This is one of the largest analyses of exercise and blood pressure yet conducted, and it complicates the aerobic-first assumption baked into older guidance. Isometric training, essentially holding a static muscle contraction, outperformed the modes usually prescribed, and the authors argue the results should inform future exercise guidelines for preventing and treating hypertension. Network rankings compare treatments across trials rather than head to head within them, so the ordering is a statistical best estimate. How isometric protocols should be standardised, and for whom they work best, remain open questions.

If you exercise partly for your blood pressure, this is a reason to know that every consistent training mode helps, and that static holds such as wall squats performed unexpectedly well in the rankings.
