Why it matters
Metabolic syndrome is a cluster of risk factors, including a large waist, raised blood fats, high blood pressure and elevated blood sugar, that together push up the risk of heart disease and diabetes. Garlic has been eaten for thousands of years and is one of the most persistent folk remedies for cardiovascular health. They included trials in adults who met the clinical picture of metabolic syndrome or had metabolic disorders, where garlic was the only intervention tested against placebo. Nineteen randomised trials with 999 participants qualified. Outcomes covered waist circumference, body mass index, triglycerides, cholesterol measures, blood pressure and fasting blood glucose, and the team ran further analyses across six factors including dose, duration and the form of garlic taken.

Compared with placebo, garlic significantly reduced triglycerides, total cholesterol, LDL cholesterol, diastolic blood pressure, body mass index and waist circumference. The remaining measured outcomes, including HDL cholesterol and fasting blood glucose, were not reported as significantly improved. Follow-up analyses suggested that participants' age and the size of the trial may modify the effect. Where it fits
Garlic joins a group of food-based interventions with genuine but modest trial evidence for cardiometabolic risk markers. The direction of effect across six separate outcomes is encouraging and consistent with garlic's long-standing reputation. However, the disagreement between trials and the possibility of missing negative studies mean the true size of the benefit remains uncertain. The authors themselves call for larger, higher-quality randomised trials before firm conclusions are drawn.
What it means for you
If you are watching your cholesterol, blood pressure or waistline, this is a reason to view garlic as a plausible supporting player rather than a fix. The measured changes were in risk markers, not in heart attacks or diabetes diagnoses, and the evidence base has real weaknesses. Nothing here suggests garlic can substitute for the fundamentals of diet, activity and any prescribed treatment. At most, it is a modest addition with a fair amount of trial evidence behind it.
