Why it matters
Cardiovascular disease and metabolic syndrome are driven by a familiar cluster of risk factors: blood pressure, cholesterol, body weight and blood sugar. Exercise reliably improves many of them, but plenty of people are drawn to gentler mind-body practices instead, and yoga is among the most popular. The open question was whether asana-based yoga produces measurable changes in these hard risk factors, or whether its benefits are limited to how people feel. This review pulled together the randomised evidence to find out, and to test yoga against both doing nothing and doing conventional exercise.
What they did
The researchers searched MEDLINE, EMBASE, CINAHL, PsycINFO and the Cochrane Central Register of Controlled Trials for randomised controlled trials through December 2013, restricted to English, peer-reviewed studies of asana-based yoga in adults reporting relevant outcomes. From 1404 records, 37 trials made the systematic review and 32 entered the random-effects meta-analysis. Two reviewers independently selected studies and rated quality with Cochrane's Risk of Bias tool. Outcomes covered body composition, blood pressure, lipids, heart rate and blood glucose measures, compared against non-exercise controls and against exercise.
What they found
Against non-exercise controls, yoga significantly improved body mass index by 0.77 kg/m2, systolic blood pressure by 5.21 mmHg and LDL cholesterol by 12.14 mg/dl, while HDL cholesterol rose by 3.20 mg/dl. Body weight fell by 2.32 kg, diastolic blood pressure by 4.98 mmHg, total cholesterol by 18.48 mg/dl, triglycerides by 25.89 mg/dl and heart rate by 5.27 beats per minute. The glucose measures were the exception: neither fasting blood glucose nor glycosylated haemoglobin changed significantly. Crucially, no significant difference emerged between yoga and conventional exercise, and one study reported an impact on smoking abstinence.
Where it fits
This analysis strengthened the case that yoga is more than relaxation: it moved the same risk factors that structured exercise moves, and by amounts that are clinically interesting if they hold up. It also punctured any claim that yoga is superior to exercise, since head-to-head comparisons found no significant difference. The authors flag real limits: small trial sample sizes, heterogeneity between studies and only moderate quality of the included trials, all of which soften confidence in the exact effect sizes. How yoga performs over longer periods, and why glucose measures stayed flat, remain open questions.
What it means for you
If yoga is the activity you actually enjoy and stick with, this is evidence it counts for cardiometabolic health rather than being a soft option: pooled trials show meaningful movement in blood pressure, cholesterol and body weight compared with staying sedentary. It is equally a reason not to expect yoga to beat a conventional exercise routine, because the trials found no significant difference between the two. The lack of change in blood sugar measures is worth noting for anyone focused on glucose control. As ever with pooled small trials, the direction is more trustworthy than the decimal points.
