Why it matters

The researchers searched five databases from inception to March 2024 and screened 4819 studies, ultimately including 23 randomised controlled trials with 1374 participants. Mean ages across the trials ranged from 46.2 to 67.0 years and 55% of participants were male. The meta-analyses compared HIIT against moderate-intensity continuous training and against non-exercise control for each component of metabolic syndrome: waist circumference, systolic and diastolic blood pressure, HDL cholesterol, triglycerides and fasting blood glucose, plus other clinically relevant cardiometabolic outcomes. What they found

Compared with non-exercise control, HIIT significantly improved every component measured: waist circumference fell by 4.12 cm on average, systolic blood pressure by 6.05 mm Hg, diastolic blood pressure by 3.68 mm Hg, triglycerides by 0.34 mmol/L and fasting glucose by 0.35 mmol/L, while HDL cholesterol rose by 0.12 mmol/L. Against moderate-intensity continuous training, HIIT was comparable across all parameters rather than superior. The subgroup finding is the practical headline: low-volume protocols, meaning under 15 minutes of high-intensity exercise per session, were not inferior to higher-volume protocols for improving the components of metabolic syndrome.

Where it fits

This review confirms what smaller bodies of evidence had suggested: interval training is an efficacious strategy for cardiometabolic health in people with metabolic syndrome, and it extends the picture by showing the benefit holds across every individual component of the syndrome. What it means for you

If a long cardio session is the reason exercise keeps not happening, this is evidence that the dose can be far smaller than assumed: sessions containing under 15 minutes of genuinely hard work were not inferior to longer protocols for improving the markers that define metabolic syndrome.
