Why it matters

High blood pressure is one of the biggest drivers of cardiovascular risk, and interest in food-derived compounds that might nudge it downwards is enormous. Grape seed extract has been tested in a number of clinical trials, but the results kept coming back inconsistent, with some trials showing reductions and others showing none. Pooling the trials is the standard way to find out whether a real effect is hiding beneath the noise. This analysis set out to do exactly that, and to identify who responds most.

What they did

Two investigators independently selected trials and extracted the data, a standard guard against error and bias. They found 12 published articles covering 16 randomised controlled trials and 810 participants in total. Changes in systolic and diastolic blood pressure after grape seed extract were pooled as weighted mean differences against control. The team also ran subgroup analyses by age, body mass index and metabolic health, plus regression analyses to see which participant characteristics predicted bigger responses, and checked for publication bias.

What they found

Overall, grape seed extract lowered systolic blood pressure by 6.077 mmHg and diastolic by 2.803 mmHg compared with control. The effect was clearest in particular groups: participants with a mean age under 50 saw systolic drops of 6.049 mmHg, those with a body mass index of 25 or above saw 4.469 mmHg, and people with metabolic syndrome saw the largest systolic reduction at 8.487 mmHg. Regression analyses showed that age, body mass index and starting blood pressure were all linked to the size of the reduction. The authors found no sign of publication bias.

Where it fits

This pooling helps explain why individual trials disagreed: the effect appears to depend heavily on who is taking the extract, with younger, heavier and metabolically unhealthy participants responding most. It fits a wider interest in dietary approaches to blood pressure, but the total evidence base remains small at 810 people. The authors themselves say a large-scale, long-term, multiple-dose trial is needed, especially among hypertensive patients. Until then, the durability of the effect and the right dose remain open questions.

What it means for you

This is a reason to think grape seed extract can genuinely move blood pressure numbers, at least over the timescales these trials ran. The average systolic drop of 6.077 mmHg would be meaningful if it holds up, but the small pooled sample means it might not. Anyone with high blood pressure should note the authors' own caution that hypertensive patients are exactly the group in whom confirmation is still needed. It is a lead worth watching rather than a settled result.
