What they did

The team searched PubMed, the Cochrane Library, Embase and Web of Science from inception to October 2021 for randomised controlled trials testing the biochemical effects of physical activity in people with mild cognitive impairment. Thirteen trials qualified, involving 514 participants in total. Between them, the trials reported on 8 inflammatory cytokines, including TNF-α, several interleukins, C-reactive protein and interferon-γ, and 5 neuroprotective factors, including BDNF, IGF-1, VEGF, FGF-2 and irisin. Results were pooled as standardised differences between exercise and control groups.

What they found

Physical activity produced significant favourable shifts in four of the markers examined. On the inflammatory side, TNF-α fell modestly and C-reactive protein fell more substantially, with the trials in reasonable agreement on both. On the protective side, BDNF and IGF-1 both rose, with the IGF-1 result showing no disagreement between trials at all. The remaining cytokines and neuroprotective factors reported across the trials did not show the same clear pattern. The authors conclude that exercise inhibits inflammatory signalling while promoting neuroprotective factors, offering a possible molecular explanation for its cognitive benefits.

Where it fits

This analysis strengthens a mechanistic story that has been assembling for years: exercise appears to act on the brain partly through the blood, damping inflammation and boosting growth-supporting molecules. It extends that story specifically to people with mild cognitive impairment, the group for whom slowing decline matters most. What it cannot do is close the loop, because these trials measured blood chemistry rather than memory or thinking, so whether the marker shifts track real cognitive change in this population remains the central open question. Larger trials linking both together would be the natural next step.

If you are watching your own memory, or a parent's, this is a reason to think of physical activity as acting on the brain's underlying environment, not just on fitness. The molecules that moved here, less TNF-α and CRP, more BDNF and IGF-1, are exactly the ones researchers implicate in protecting ageing brains. That said, the honest reading is one step short of the headline everyone wants: this shows exercise changes the chemistry, not that it demonstrably sharpened anyone's thinking in these trials. It adds mechanism to an already encouraging picture rather than settling it.
