Why it matters

Aerobic exercise is one of the most commonly prescribed non-drug approaches to obesity, and the assumption behind it is simple: burn more calories, lose more weight. Yet how much weight cardio actually removes when it is the only intervention, with no diet attached, has never been as clear as the advice suggests. This review set out to quantify exactly that, pooling randomised trials in overweight and obese adults where aerobic exercise stood alone, to establish what the treadmill, unaided, is really worth on the scale.

What they did

The authors searched for published randomised controlled trials of aerobic exercise up to 20 January 2010, including only those where the exercise programme was an isolated intervention rather than part of a diet or multi-component package. Fourteen trials qualified, covering 1847 overweight and obese participants, with programmes running from 12 weeks to 12 months. Results were combined with a random-effects model, pooled separately for programmes lasting 6 months and those lasting 12 months, with body weight and waist circumference as the key outcomes.

What they found

Six-month aerobic programmes produced an average weight loss of 1.6 kg and trimmed waist circumference by 2.12 cm. Twelve-month programmes fared little better despite doubling the duration: 1.7 kg of weight and 1.95 cm off the waist. The authors describe these effects as modest and conclude that isolated aerobic exercise is not an effective weight loss therapy for overweight and obese patients. They note, however, that the exercise still delivered modest benefits to blood pressure and blood lipids, and that it may work as a weight loss therapy when combined with dietary change.

Where it fits

This pooled analysis put firm numbers on something exercise scientists had long observed: people lose far less weight from exercise alone than simple calorie arithmetic predicts. It reframes cardio's role in weight management, shifting the emphasis toward diet as the primary driver of weight loss with exercise as a health-improving companion. The review covered moderate-intensity aerobic programmes only, so it cannot speak to higher intensity formats or resistance training. Whether different training approaches would change the picture is a question this evidence could not answer.

What it means for you

If you have been running or cycling for months and the scale has barely moved, this review suggests that is the norm rather than a personal failure: on average, a full year of cardio alone bought 1.7 kg. That is not a reason to stop, because the same trials showed benefits to blood pressure and lipids that have nothing to do with weight. It is a reason to recalibrate expectations: if weight loss is the aim, the evidence here points to combining exercise with dietary change rather than relying on exercise to do the work alone.
