Why it matters
Phone apps, text nudges and digital coaching promise to get people moving at almost no cost per person, which is why health systems and insurers are so interested in them. Plenty of trials show they work while the programme is running, but behaviour change is notoriously good at evaporating once the prompts stop. The question that actually matters for anyone downloading a fitness app is whether the extra activity survives after the study ends. This review set out to measure both the immediate effect and what remains months later, and to ask who benefits most.
What they did
The authors searched PubMed, the Cochrane Library, SCOPUS and PsycINFO in July 2020 for randomised controlled trials of mobile health interventions targeting physical activity as a primary outcome in adults. From 2828 identified studies, 117 were included, reporting on 21,118 participants with a mean age of 52.03 years, of whom 58.99% were women. They extracted results at the end of intervention, at follow-ups up to 6 months, and at follow-ups beyond 6 months, across four outcomes: walking, moderate-to-vigorous activity, total physical activity and energy expenditure. Subgroup analyses explored population, intervention design and control group type.
What they found
At the end of the intervention, mobile health programmes significantly increased all four outcomes, with the largest standardised effects for walking at 0.46 and energy expenditure at 0.44. Only 33 studies reported short-term follow-up and just 8 reported long-term follow-up; in the long term, effects were sustained for walking at 0.25 and moderate-to-vigorous activity at 0.19, both smaller than the immediate effects. Effects were larger in sick and at-risk populations, and appeared regardless of whether the design was scalable or the control group type. The risk of bias was rated high in 80.3% of studies, and the quality of evidence was low to very low.
Where it fits
This is one of the larger syntheses of digital activity interventions, and it strengthens the case that they produce genuine, if modest, increases in movement rather than just enthusiasm during the study. The finding that effects persist beyond 6 months, while shrinking, is more encouraging than the common assumption that digital nudges stop working the moment they stop arriving. The very small number of trials with long follow-up, and the high risk of bias in most studies, mean the durability estimate rests on thin ground. The authors call for methodologically rigorous trials focused specifically on long-term effects.
What it means for you
If you use an app or wearable prompts to stay active, this is a reason to believe the approach can produce real changes in how much you walk and move, not just a placebo of good intentions. It is equally a reason for realistic expectations: the effect was small to moderate at its peak and smaller months later, so the app is a nudge, not an engine. The larger effects seen in at-risk and unwell populations suggest the people with the most room to improve gained the most. Whatever the technology, the durable part is the habit it helps you build.
