Tiny doses of HIIT raise fitness but shift no body fat

Why it matters
High-intensity interval training is sold on the promise that a few hard minutes can stand in for a long session, and much of that promise is implicitly about body shape rather than heart and lung fitness. Individual trials have been small and inconsistent, and many lumped genuinely brief protocols in with far larger training doses. That left an open question: does a very small weekly volume of intervals change fat mass and lean mass at all, or only fitness? It matters because lack of time is the most common reason people give for not exercising, and because being fitter and being leaner are not the same outcome.
What they did
The authors searched six databases from the earliest records to June 2019, screened 11,485 records and included 47 controlled studies, both randomised and non-randomised, each lasting at least 4 weeks. Low-volume HIIT was defined precisely as training totalling no more than 500 metabolic equivalent minutes per week. Participants were adults of normal weight, overweight and obese. Results were pooled for two comparisons, low-volume HIIT against a non-exercising control and against moderate-intensity continuous training, across total body fat mass in kilograms, body fat percentage, lean body mass in kilograms and cardiorespiratory fitness.
What they found
On body composition, nothing moved. Against no exercise, low-volume HIIT showed no significant difference in total body fat mass, body fat percentage or lean body mass, and the same held when it was compared with moderate-intensity continuous training. Most individual studies leaned towards better lean body mass with low-volume HIIT than with continuous training, but once pooled that difference was not significant, which the authors flag as a novel result. Cardiorespiratory fitness was the clear exception, improving significantly more with low-volume HIIT than with no exercise (p < 0.001) and more than with continuous training (p = 0.017), despite the smaller training volume.
Where it fits
This tempers a popular claim without dismantling it. The fitness side of the HIIT story survives intact, and arguably looks better than before given how little training was needed to beat longer moderate sessions. The body composition side does not: a short, hard weekly dose on its own did not reliably strip fat or add lean tissue in these trials. Open questions include whether longer interventions, higher volumes or intervals combined with diet change would produce the fat loss that low-volume work alone did not, and how much the mix of randomised and non-randomised studies shaped the pooled picture.
What it means for you
This is a reason to judge short interval sessions by what they demonstrably deliver, which here was fitness. If your goal is a smaller waist, the evidence pooled here suggests intervals by themselves are unlikely to be the lever that moves it. It also means the fear that hard, brief cardio eats muscle was not borne out: lean mass did not differ from either comparison. Fitness itself is worth having, and the finding that a small dose can raise it more than a longer moderate session is genuinely useful information for anyone short on time.
