Why it matters

The researchers searched the SCOPUS, PubMed, EBSCO and LILACS databases for trials published up to June 1, 2022 that tested microbiome therapies against controls in people with type 2 diabetes and measured waist circumference. Fifteen reports qualified, covering 443 treated participants and 387 controls. Results were pooled using a random effects model as differences in means, with separate analyses by therapy type. Blood sugar outcomes, including HbA1c, fasting glucose, fasting insulin and insulin resistance, were analysed alongside the waist measurements.
What they found
Overall, microbiome therapies reduced waist circumference by a statistically significant but very small 0.97 cm. When split by therapy type, only probiotics held up, at 0.62 cm, while prebiotics and synbiotics showed no waist effect at all. The blood sugar picture was stronger: HbA1c, fasting glucose and insulin resistance all fell significantly with microbiome therapies overall, and with probiotics specifically. There were wrinkles, though: prebiotics lowered HbA1c and fasting glucose but raised fasting insulin, and synbiotics were associated only with a rise in fasting insulin. Single-strain and multi-strain formulations performed no differently.

This analysis converts an inconclusive literature into a clear, if deflating, answer on waistlines: the effect exists but is too small to matter to any individual. It sits alongside broader evidence that gut-targeted supplements influence metabolic markers more readily than body shape. The unexpected rises in fasting insulin with prebiotics and synbiotics complicate the simple story that all gut supplements help, and deserve explanation. Which strains and doses drive the probiotic benefits, and whether the glycaemic improvements persist long term, remain open questions.
What it means for you
If you have type 2 diabetes, or are working to avoid it, and have seen probiotics marketed for belly fat, this is the honest sizing: under a centimetre on average, which you would never notice. The more interesting signal is in blood sugar control, where probiotics were consistently associated with improvements. It is also worth knowing that the therapy types are not interchangeable, since prebiotics and synbiotics moved fasting insulin in the wrong direction here. None of this replaces whatever regime you and your clinician have settled on.
