They found 54 trials covering 60 intervention groups and 3390 participants in total. Outcomes included fasting glucose, glucose measured after a meal, fasting insulin, HbA1c and insulin resistance, alongside inflammatory markers and the make-up of the gut bacteria themselves. Subgroup analyses examined region, type of therapy, number of probiotic strains, dose, duration, average age and baseline body mass index.

Fasting glucose fell by an average of 7.97 mg/dL, and glucose measured two hours after a meal dropped by 43.30 mg/dL. HbA1c, the longer-run marker of blood sugar control, fell by 0.28%, and fasting insulin and insulin resistance scores also improved. Inflammatory markers moved in a favourable direction: high-sensitivity C-reactive protein, tumour necrosis factor alpha and lipopolysaccharides fell, while the anti-inflammatory interleukin-10 rose. This is one of the larger pooled analyses of microbiome-targeted therapies in type 2 diabetes, and it lands on a consistent if modest benefit across several glucose measures at once. However, the included trials disagreed with each other, and long-term data are limited, so the durability of the effect is unknown. The authors explicitly call for large-scale trials before firmer conclusions are drawn.
