The researchers systematically searched Web of Science, Scopus, PubMed and EMBASE using relevant keywords up to August 2023, looking for randomised controlled trials of Nigella sativa in people diagnosed with metabolic syndrome and its associated conditions. Sixteen RCTs met the inclusion criteria, comprising 1033 participants in total. The trials compared Nigella sativa intake against placebo, and the meta-analysis pooled effects on inflammatory biomarkers, C-reactive protein (CRP), TNF-α and IL-6, alongside markers of oxidative stress, including malondialdehyde (MDA), superoxide dismutase (SOD) and total antioxidant capacity (TAC).
What they found
Compared with placebo, Nigella sativa significantly reduced CRP (standardised mean difference -0.60), TNF-α (-0.53) and IL-6 (-0.54), pointing to a consistent anti-inflammatory effect across trials. On the oxidative-stress side, MDA, a marker of oxidative damage, fell substantially (SMD -1.28), while the antioxidant measures moved upward: SOD rose (SMD 1.35) and total antioxidant capacity increased (SMD 2.82). Every one of these pooled effects reached statistical significance, though some confidence intervals were wide, and the analysis reports biomarker changes only: no clinical events or symptoms were assessed.
Where it fits
This pooling turns a scatter of individual trials into a more coherent picture: black seed appears to shift both inflammatory and oxidative-stress markers in a favourable direction in people with metabolic syndrome and related disorders. The authors phrase their conclusion cautiously, consumption 'could be associated' with improvement, which is appropriate given the endpoints are blood markers rather than disease outcomes. Open questions remain around the best dose, preparation and duration, whether effects hold in healthy people, and above all whether biomarker improvements of this size translate into fewer cardiovascular events or better metabolic health over time.

If you have filed black seed under folklore, this is a reason to update slightly: across 16 randomised trials it reliably moved inflammation and oxidative-stress markers in people with metabolic syndrome and related conditions.
