Why it matters

Omega-3 long-chain polyunsaturated fatty acids are among the most widely taken supplements, and one of the claims attached to them is that they promote healthy sleep. Individual trials testing that claim have come back inconclusive, with some suggesting benefit and others finding nothing, which leaves anyone taking fish oil partly for better sleep without a clear answer. Sleep matters across the whole lifespan, from infant brain development to adult health, so the question of whether a cheap, common supplement can improve it is worth settling. This review set out to pool everything available, across trials and studies that tracked people's intake over time, and across every age group studied.

What they did

The researchers systematically searched five databases, PubMed, EMBASE, the Cochrane Library, CINAHL and AMED, from their inception up to November 2019. They included randomised controlled trials, clinical trials with a control group, and longitudinal studies that reported omega-3 intake alongside sleep-related outcomes. Twenty studies made the cut: 12 clinical trials and 8 longitudinal studies. Each study was appraised for quality using the Cochrane risk-of-bias tools and the Newcastle-Ottawa Scale, and the results were pooled as weighted mean differences using fixed or random effect models, covering infants, children and adults separately.

What they found

For adults, the headline was a clean null: no effect on total sleep duration, sleep latency, sleep efficiency, sleep quality or insomnia severity. The picture in early life was different. In infants, omega-3 reduced the percentage of active sleep by 8.40 points and sleep-wake transition by 1.15 points, and increased the percentage of wakefulness by 9.06 points, though it had no effect on quiet sleep; the authors read this as improved sleep organisation and maturity. In children with clinical-level sleep problems, total sleep disturbance scores fell by 1.81 points, but healthy children saw no change in total sleep duration, sleep latency or sleep efficiency.

Where it fits

Before this review, the trial evidence on omega-3 and sleep was openly described as inconclusive, so pooling it adds real clarity: the apparent benefits concentrate in early childhood and in children who already have sleep problems, while the adult evidence points nowhere. That complicates the popular framing of fish oil as a general sleep aid. It also leaves open questions the abstract itself flags: the authors call for additional robust studies to confirm the relationship, and the review cannot say why effects would appear in infancy but vanish by adulthood, or whether different formulations or durations would change the adult picture.

What it means for you

If better sleep is the main reason an adult reaches for fish oil, this pooled evidence offers no support for that particular hope: every adult sleep measure tested came back unchanged. That is not a verdict on omega-3's other claimed benefits, which this review did not examine. For parents, the findings are more nuanced: the improvements in infants relate to sleep organisation and maturity rather than simply more sleep, and the benefit in children applied to those with clinical-level sleep problems, not to healthy children. As ever with pooled evidence that the authors themselves want confirmed, this is a reason to recalibrate expectations rather than a final word.
