Resistance bands alone cut blood pressure, blood sugar and body fat in older adults

Why it matters

Metabolic syndrome is the cluster of high blood pressure, raised blood sugar, unfavourable cholesterol and excess body fat that drives heart disease and type 2 diabetes, and it becomes more common with age. Conventional resistance training can improve these markers, but gyms, machines and heavy loads are not accessible or appealing to every older adult. Elastic bands are cheap, portable and easy to scale, which makes them an attractive option for sedentary people over 65. What has been missing is a clear, pooled picture of whether band training actually moves the numbers that matter.

What they did

The researchers pooled randomised controlled trials comparing elastic band resistance training against non-active controls in physically independent, sedentary adults aged 65 or older who had metabolic syndrome or were at cardiometabolic risk. They searched five major databases through September 2024 and assessed each trial for risk of bias using established tools. Twenty-three trials met the criteria, covering 1,134 participants with an average age of 70.4, and 92.4% of them were women. Outcomes included body composition, blood pressure, lipid profile and fasting glucose, and meta-regressions explored which programme features tracked with greater fat loss.

What they found

Band training improved every component measured. Body fat percentage fell by the equivalent of 1.44 percentage points and fat mass by around 1.57 kg compared with controls. Total cholesterol dropped by 18.21 mg/dL, LDL by 11.03 mg/dL and triglycerides by 11.98 mg/dL, while HDL, the protective fraction, rose by 2.42 mg/dL. Fasting glucose fell by 9.02 mg/dL, systolic blood pressure by 7.73 mmHg and diastolic pressure by 5.45 mmHg. The links between specific programme characteristics and fat loss were exploratory, and the degree of variation between studies differed by outcome.

Where it fits

The authors describe the improvements as clinically meaningful for older adults at risk of metabolic syndrome, which places band training alongside more conventional resistance exercise as a credible tool for cardiometabolic health. Publication bias appeared minimal, which strengthens confidence in the direction of the findings. That said, the sample was overwhelmingly female, so how well the effects generalise to older men remains unclear. The meta-regression findings about which programme features drive fat loss are explicitly exploratory and are intended to shape future trials rather than settle the question.

What it means for you

For anyone over 65, or helping a parent who is, this is a reason to take resistance bands seriously rather than treating them as a lesser substitute for weights. The trials were run in sedentary, physically independent older adults, so the findings speak most directly to people starting from little or no training. The improvements spanned blood pressure, blood sugar, cholesterol and body fat at once, which is unusual breadth for one low-cost intervention. It does not show bands beat other forms of exercise, only that they clearly beat doing nothing.
