Pooled analysis suggested a modest reduction in inflammatory lesion counts favouring probiotics, with a standardised mean difference of -0.57. The catch is that the trials disagreed with each other substantially, and the 95% prediction interval ran from -1.25 to 0.11, meaning a future study could plausibly find no meaningful effect. Sensitivity analyses using an alternative statistical approach produced comparable results. All three trials reported good short-term tolerability with no serious adverse events. Risk of bias was low in two trials and of some concern in one, and the certainty of evidence was rated low to moderate.
Where it fits
This is one of the more rigorous looks at oral probiotics for acne, restricted to double-blind randomised trials rather than the looser designs that often pad out this literature. It supports the idea of probiotics as a generally safe, antibiotic-sparing adjunct, but it also exposes how little solid evidence exists: three trials is a very narrow base, and the inconsistency between them leaves the true size of any effect uncertain. The authors call for larger, standardised trials before any firm clinical recommendation can be made.
What it means for you
If you deal with acne and dislike the idea of long antibiotic courses, this is a reason to keep an eye on probiotic research rather than a reason to expect results from a capsule today.
