Probiotics eased eczema severity scores in adults, but the itch stayed put
In pooled results from six randomised trials in 241 adults with atopic dermatitis, probiotics significantly lowered overall SCORAD severity scores and improved quality of life. Itch severity, skin severity, a standard dermatology life-quality index and immune markers showed no significant change.

Key takeaway
What it shows: A mixed picture worth holding loosely: only six trials with 241 adults, the studies disagreed with each other heavily, and the itch itself did not improve even as overall severity scores fell. None of the trials reported side-effect data.

Why it matters
Atopic dermatitis is a chronic, relapsing inflammatory skin disease that can seriously disrupt the quality of life of the people it affects. Because the immune system drives the condition, there has long been interest in whether probiotics, which can act as immunomodulators and enhance immune control of the disease, might calm it from the inside. As the authors note, probiotics have been widely studied in atopic dermatitis with controversial results, so this review pooled the randomised trials conducted specifically in adult patients to see where the evidence actually stands.
What they did
The authors searched the PubMed database and performed manual searching for randomised controlled trials of probiotics in the treatment of adult atopic dermatitis published before December 2020. Six trials qualified, together enrolling 241 patients of Asian and European origin. The pooled outcomes included the SCORAD index, a standard composite measure of eczema severity, alongside skin severity, itch severity, quality-of-life measures including the Dermatology Life Quality Index, and immune markers including IL-4, TNF-γ and IgE.
What they found
Probiotics were linked to a significant drop in SCORAD of 7.90 and a significant improvement in quality-of-life scores of 7.68. Beyond those two results the picture went quiet: skin severity, itch severity, the Dermatology Life Quality Index and the immune markers IL-4, TNF-γ and IgE all showed no significant difference between probiotics and control. The trials also disagreed heavily with each other on the SCORAD result, and none of the included studies provided any data on side effects of the probiotics used.
Where it fits
The review lands in the middle of a genuinely contested area, and its own results reflect that. A fall in the composite SCORAD score alongside no change in itch or in the standard dermatology life-quality index is an awkward combination, suggesting that any benefit may be narrower than the headline severity result implies. With only six trials, 241 patients, considerable disagreement between studies and a complete absence of safety data, the finding is a starting point for better trials rather than a settled answer on probiotics for adult eczema.
What it means for you
If you are an adult with eczema, this is weak but real evidence that probiotics might nudge overall disease severity and quality of life in the right direction. It is equally important to know what did not move: the itch, which for most people is the symptom that matters, showed no significant improvement, and neither did the measured immune markers. With no side-effect data collected in any of the trials, the honest reading is cautious curiosity rather than confidence.
