Why it matters
Insulin resistance sits upstream of type 2 diabetes and much of modern metabolic disease, and diet is the most accessible lever for changing it. Plant-based diets are widely promoted for metabolic health, but the claims often run ahead of the randomised evidence, and it has been unclear whether the benefits apply equally to people with overweight, prediabetes and established type 2 diabetes. This review set out to pool the trial evidence on plant-based eating and markers of insulin sensitivity across those three groups.
What they did
The authors systematically searched MEDLINE, Embase, CINAHL and CENTRAL for randomised controlled trials of plant-based diets, spanning vegan, ovo-vegetarian, lacto-vegetarian and lacto-ovo-vegetarian patterns, lasting at least 14 days. Eligible participants were adults with a BMI of 25 or above, prediabetes, or type 2 diabetes, and the outcomes were markers of insulin sensitivity. Eight trials with a combined 716 participants met the criteria, and results were pooled against control diets where possible.
What they found
In people with overweight or obesity, plant-based diets improved HOMA-IR by a pooled 0.97 and lowered fasting insulin by 4.13 µU/mL compared with control diets, both statistically significant. The picture was different elsewhere: one study in people with prediabetes compared vegan and vegetarian diets and found no difference in HOMA-IR or fasting insulin, and one study in people with type 2 diabetes reported no difference in insulin measures or glucose clearance against a conventional diabetes diet. The clear signal, in other words, was specific to the overweight and obesity group.
Where it fits
This supports the popular claim that plant-based eating improves insulin handling, but with sharper boundaries than the marketing suggests: the pooled benefit was demonstrated in overweight and obesity, while the evidence in prediabetes and type 2 diabetes rests on single studies showing nothing. The interventions were also short, some as brief as 14 days, so the durability of the improvements is unknown. The authors explicitly call for long-term randomised trials to test whether the gains persist in people at risk of or living with type 2 diabetes.
What it means for you
If you carry extra weight and are curious about plant-based eating, this is evidence that measurable improvements in insulin sensitivity can appear within a fortnight of switching. It is not evidence that a plant-based diet outperforms a conventional diabetes diet if you already have type 2 diabetes, where the single available trial found no difference. The fair summary is that plants moved the metabolic needle quickly in the group most of the trials studied, and the long-term picture is still being written.
