Weights, not just cardio, lower cholesterol after menopause

Why it matters

Cardiovascular risk climbs after menopause, and unfavourable blood lipids are one of the main routes it travels. Exercise is a first-line answer, but most of the attention has gone to aerobic work, and whether resistance training on its own can move cholesterol and triglycerides in postmenopausal women has remained genuinely unclear. Individual trials pointed in different directions. This review pooled the randomised evidence to clarify what lifting actually does to total cholesterol, LDL, HDL and triglycerides in this group.

What they did

The authors searched Web of Science, Scopus, PubMed and Embase for randomised controlled trials that tested resistance training in postmenopausal women and measured blood lipids. Nineteen trials qualified. Their results were pooled using a random effects model, and subgroup analyses cut the data by age, the length of the intervention, lipid levels before enrolment and body mass index, to see where the training made the biggest difference.

What they found

Resistance training lowered total cholesterol by 11.47 mg/dl, LDL cholesterol by 8.48 mg/dl and triglycerides by 6.61 mg/dl on average. The effects were larger in women under 60 years old, in trials lasting under 16 weeks, and in women who began with high cholesterol or obesity; women whose LDL was 130 mg/dl or more before enrolment saw a 14.38 mg/dl drop in LDL. The exception was HDL cholesterol, which fell slightly, by 2.97 mg/dl, and only in women with obesity.

Where it fits

The result strengthens the case that resistance training is a cardiovascular intervention rather than only a muscular one, and it extends that case specifically to postmenopausal women, a group whose lipid profiles often worsen. Two details complicate the tidy story. Shorter programmes produced larger changes than longer ones, which is not what steady adaptation would predict, and the small HDL drop in women with obesity runs in the unhelpful direction. Both patterns need explaining before firm guidance can be built on them.

What it means for you

If you are postmenopausal and your cholesterol has been creeping up, this is a reason to think the weights room belongs in the conversation alongside walking, cycling and diet. The pooled effects were meaningful rather than dramatic, and they were biggest in the women who had the most room to improve. Treat it as information for understanding your own numbers, not a replacement for anything a doctor is already managing.
