Why it matters

Resistance training is one of the best-evidenced ways for older adults to build muscle and push back against age-related decline, but life intervenes: illness, injury, travel and waning motivation all interrupt training. What happens to that hard-won muscle when the training stops is a question with real consequences for how older people plan their exercise across the year. This review set out to quantify the effect of resistance training cessation — detraining — on muscle size specifically in older adults, and to ask whether the length of the layoff changes how much is lost.

What they did

Five databases were searched for eligible studies of resistance training followed by a period of training cessation in older adults. Six studies, contributing eight groups, met the criteria. The resistance training interventions lasted from 9 to 24 weeks, and the subsequent detraining periods ranged from 12 to 52 weeks. Methodological quality was assessed with the PEDro checklist, with studies rated fair or good, and the data were pooled in a random-effects meta-analysis comparing muscle size at baseline, after training, and after detraining.

What they found

Resistance training worked as expected: muscle size increased significantly from baseline (Cohen's d 0.99; 95% CI 0.63 to 1.36). After training stopped, muscle size decreased significantly compared with the post-training values (Cohen's d -0.83; 95% CI -1.30 to -0.36). Crucially, the subgroup analyses split by layoff length: after 12-24 weeks of cessation the decrease was not statistically significant (Cohen's d -0.60; 95% CI -1.21 to 0.01), whereas after 31-52 weeks the loss was significant and larger (Cohen's d -1.11; 95% CI -1.75 to -0.47).

Where it fits

The 'use it or lose it' principle is widely assumed, but this analysis puts numbers on it for the population where muscle loss matters most. It confirms that training gains in older adults are reversible, while adding the more nuanced finding that the loss appears related to detraining duration, with longer layoffs producing clearly greater shrinkage. With only six studies pooled, the picture is coarse: the review itself calls for future work to establish the precise time course of muscle size changes during detraining, and it does not address how quickly retraining restores what was lost.

What it means for you

For older lifters, this is a reason to see consistency, not perfection, as the goal. Muscle gained through resistance training does erode when training stops entirely, but in these pooled data the loss only became statistically clear over the longer layoffs of roughly seven months to a year, not over shorter breaks of three to six months. An interrupted year of training, in other words, looks meaningfully different from an abandoned one — though the small evidence base means the exact timeline of loss is still uncertain.
