Why it matters
Chromium picolinate is one of the most widely sold weight-loss supplements, marketed as a slimming aid in the USA, Europe and across the internet. It has also been advocated in the medical literature for reducing body weight, which gives it an air of legitimacy that most slimming products lack. With obesity described as a global public health threat, a supplement that genuinely helped people lose weight would matter enormously. This Cochrane review set out to establish whether the evidence actually supports the claims made for it in overweight and obese adults.
What they did
The reviewers searched major international and Chinese databases up to December 2012, plus trial registers and reference lists, for randomised controlled trials of chromium picolinate in overweight or obese people. Studies in children, pregnant women and people with serious medical conditions were excluded. Nine trials involving 622 participants met the criteria, all conducted in community settings with follow-up of up to 24 weeks. Three trials combined the supplement with resistance or weight training against placebo plus the same training; the rest tested the supplement alone against placebo, across doses of 200 μg, 400 μg, 500 μg and 1000 μg.
What they found
Across all doses pooled together, chromium picolinate produced an average of 1.1 kg more weight loss than placebo after 12 to 16 weeks of treatment, based on 392 participants in six trials, an effect the authors described as of debatable clinical relevance and graded as low-quality evidence. No dose gradient could be established, and no firm evidence emerged for body mass index, body fat percentage or waist circumference. Only three studies reported on side effects at all: two serious adverse events occurred at the 1000 μg dose and one at 400 μg, while two placebo participants also dropped out after adverse events. No study reported on mortality, illness, quality of life or costs.
Where it fits
This is the kind of comprehensive, independent synthesis that marketing claims rarely survive, and chromium picolinate did not fare well. The pooled effect technically favoured the supplement, but the reviewers judged the evidence too weak and too low quality to inform any firm decision about efficacy or safety. That leaves the gap between how the product is sold and what the science shows unusually wide. Open questions include whether any dose is meaningfully effective and what the longer-term safety profile looks like, since no trial ran beyond 24 weeks.
What it means for you
If you have seen chromium picolinate promoted as a fat-loss shortcut, this review is a reason to treat those claims with real doubt. Even taking the pooled result at face value, the entire effect amounts to roughly a kilogram over three to four months, and the reviewers did not consider the underlying evidence reliable. The safety picture is similarly unclear, with serious adverse events reported but too little data to judge. This is a case where the strength of the marketing considerably outruns the strength of the evidence.
