Calcium and vitamin D do more for your bones when you exercise too

Why it matters

Exercise and calcium with vitamin D are both standard preventive measures, yet they are often discussed separately, and it has never been clear whether combining them delivers more than the supplements alone. The reviewers systematically searched eight databases for studies published through October 2025 and included 13 randomised controlled trials in postmenopausal women. Each trial compared exercise combined with calcium and vitamin D supplementation against the supplements alone, with bone mineral density as the outcome. The team assessed each trial's risk of bias with an established tool, graded the overall certainty of the evidence, and ran subgroup, sensitivity and meta-regression analyses to probe why results differed between studies and whether the findings held up.

What they found

Combined interventions significantly increased bone mineral density at the lumbar spine (a standardised mean difference of 0.31) and the femoral neck (0.47) compared with supplements alone, which are the two sites where fractures matter most. Other skeletal sites showed consistent trends in the same direction that did not reach significance. In the subgroups, whole-body vibration produced the largest and most consistent benefits at both key sites, while mind-body and traditional Chinese exercises such as Baduanjin significantly improved lumbar spine density. Curiously, interventions lasting six months or less produced greater gains, with longer durations offering no additional advantage.

Where it fits

Exercise and calcium with vitamin D are each established measures on their own, but the evidence for genuine synergy had been unclear until now. This pooled analysis supports the idea that movement adds measurable bone benefit on top of supplementation rather than merely duplicating it. It also complicates the picture in one respect: the finding that shorter programmes outperformed longer ones runs against the usual assumption that bone adapts slowly, and needs explaining. The variety of exercise styles across trials leaves open which approach is genuinely best.

What it means for you

For women past menopause, this is a reason to think of supplements as one half of a bone strategy rather than the whole of it. The trials suggest the combination moves density at the spine and hip, exactly where fragility fractures do their damage. It is also worth noting that the standout approaches were not extreme: vibration platforms and gentle mind-body practice both registered benefits. None of this replaces individual medical advice on osteoporosis risk, but it reframes what the supplements alone can be expected to achieve.
