Why it matters

Bone mineral density falls after menopause as oestrogen declines, raising fracture risk, and both exercise and soy isoflavones are widely promoted for bone health at this stage of life. Isoflavones are plant compounds with weak oestrogen-like activity, which underpins the idea that they might slow bone loss. A natural question is whether combining them with exercise training produces more benefit than either approach alone, since many postmenopausal women do both at once. This review pooled the randomised trials that tested exactly that combination.

What they did

Two reviewers searched Medline and PubMed records up to 31 January 2019 for randomised controlled trials of isoflavone supplementation combined with exercise training in postmenopausal women, with bone mineral density as the outcome. Four trials with a total of 609 participants met the criteria. Risk of bias was classified using the Cochrane Collaboration tool, and results were pooled as standardised mean differences across several skeletal sites, including the lumbar spine, total hip, femoral neck, trochanter and ward's triangle.

What they found

The combination of isoflavones and exercise did not significantly improve bone mineral density compared with placebo at any site examined: sub-whole body, lumbar spine, total hip, femoral neck, trochanter and ward's triangle all showed effects close to zero. Nor did the combined intervention provide any additive benefit over exercise alone or isoflavone supplementation alone. The authors note that the included trials were small and that some had methodological limitations, which tempers how firmly this null result can be read.

Where it fits

This complicates the appealing logic that stacking two bone-friendly interventions should compound their effects, at least as measured by bone density. It does not settle whether isoflavones alone have any effect, nor whether longer or larger trials might detect small changes; with only four small trials pooled, the evidence base is thin. Fracture outcomes, doses, exercise types and treatment durations all remain open questions that this analysis could not address.

What it means for you

For a postmenopausal reader already exercising for bone health, this offers no evidence that adding an isoflavone supplement improves bone density further, and no evidence that pairing the two beats either one on its own. Given the small and imperfect trials behind the result, it is best read as an absence of demonstrated benefit rather than proof of no effect. Exercise itself was not on trial here; the question was purely whether isoflavones add anything on top of it.
